Was filling out a doctor's set of questionnaires this weekend which are to be turned in upon date of appointment. I'm sure ya'll have filled these out before.
However, this particular Doctor, has the most thorough set of documents I've ever seen.
On one form he asks for the patient's RACE and on separate line ETHNICITY. Under each question about race and ethnicity the form states in bold type that, "this information is now required by the U.S. Government".
So I'm not sure about this being an OBAMA CARE mandate...but I suspect so since this is the first time I've ever been required to list "ethnicity".
Well, my appointment is tomorrow and guess I'll find out.
BTW, my responses???

Ethnicity: Southern American

| This is the same crap as the census. It is none of the government's business and, frankly, the more the government WANTS to know, the more suspicious I am of WHY it wants to know! Unless one can equate a condition with one's race or "ethnicity" - and such DO exists such as cycle cell anemia and various conditions applicable to those with a Mediterranean background - what my ancestry is ain't nobody's business. |

Brother Hamp your post reminds me of a similar health questionaire I was required to complete six months prior to my retirement from the ANG. Non-flying members of the Air Force are and have been required to have periodic physicals every five years. In the years between physicals, members are and have been required to complete a simple health questionaire which until FY 2011 was a paper form that asked questions relevant to one's health, i.e. any surgery, weight gain/loss, meds taken, etc. since last periodic physical.
In FY 2011, the Air Farce, put these PHA questionaires into digital format, created a central website gathering point and all members now complete them 'online'. A number of never before asked categories such as 'Firearms Ownership', 'Reproductive Health', that have absolutely no bearing on anyone's physical ability to serve, were introduced. Being a computer based application, all categories required a response and each response triggers whether or not additional questions are asked. Failure to comply with the requirement to complete is tied to the member's performance appraisals, compensation and future.

A similar side of the same coin is the fact that any VA PTSD Disbility Claim suppported by a "PTSD Worksheet" could put the applicant on the Empire's watch list.
These standard "worksheets" ask very personal psychological questions wich, if answered truthfully, trigger automatic flags. They include any references to violence, guns, etc. as well as "thoughts" of suicide.
To me, this is the most agregious offense the Empire could perpetrate against its returning warriors. Tell the truth, potentially loose your God give rights of self defense. Nice.

| Since according to our current administration being a veteran is in itself enough to make one a suspect, anyone filling out a VA form of any type is probably already under special attention. |

This is not VA but Dep't. AF, Surgeon General and applies to all active duty AF, AF Reserve and Air National Guard members. For the last probably 10 years all members have had to attend various forms of mandatory 'Suicide Prevention' training ranging from a 30 minute computer based death by power point to all day workshops. I commented on more than one occasion that I did not require suicide prevention but was beginning to feel the need for some sort of homicide prevention. I don't want to make light of the fact that there has been a rise in the number of service members who have committed suicide post deployment, but I will venture to say that the numbers do not proportionately exceed the numbers associated with post Viet Nam, Korea, WWII or WWI for returning vets who took their own lives.
As far as 'Watch Lists', I find it ironic that in 2006 Gov Napolitano, sent me some sort of AZ community service ribbon for my participation in Operation Jump Start, for sitting on the AZ-Mex border and watching the cartels logistics system supply drugs to the US and then in 2009, DHS Sec Napolitano labels me and hundreds of thousands of military members as 'high risk' domestic terrorists.
It is customary to receive a certificate of appreciation from the sitting POTUS upon one's military retirement (it has a DoD form number). I specifically requested I not receive said certificate and for some reason I thought that at the end of 36 1/2 years service, my request might be honored. In Dec, the GPO was out of the certificates and informed me of same. In June I received a manila envelope with the White House return address affixed. I wrote RTS on it, placed it back in the mail and informed the Mrs. that if the SS came by to not speak with them and to summon the local sheriff's office. I'm sure I've made another US Guvmint 'Watch List'. |
